Hand write the patient name, date of birth, zip code and the order number (from your order confirmation email), to the side of the prescription when you fax it. Scan and email your prescription Get your written prescription from your doctor, scan it into your computer and email it to us at orders@ochiwear.com. Include the patient name, date of birth, zip code and the order number (from your checkout email) when you email us the scan. What if I don't have my prescription?
